The following vaccines will be administered during your child’s well exam appointment:

• 15 Months: Prevnar 13, Pedvax,
• 18 Months: Infanrix , Hepatits A #1
• 2 Years: Hepatitis A #2

**LEAD LEVELS WILL BE CHECKED DURING THIS VISIT**

• 3 Years: catch up of any missed vaccines
• 4 Years: Kinrix, Proquad
• 5-6 Years: catch up of any missed vaccines

During your child’s well examination, Dr. Uy may perform certain procedures that are vital part of your child’s annual check up. These services are necessary to ensure that your child receives the best care possible and to monitor your child’s growth and development. Your child’s well being is our greatest concern, therefore, we will provide only those services that will benefit your child during their well care.

Dr. Uy follows the guidelines established by the American Academy of Pediatrics which require certain assessments to remain as part of all age appropriate routine physicals. There are certain developmental, emotional and behavioral screening assessments that are necessary for us to access, score and make a medical determination based upon your child’s answers.

These screenings are essential in helping us determine many issues that may be affecting your child.

In addition to these screenings, the following testing will also be performed: Hearing screen, Vision Screen, Urine Screen.
